1) Complete the form by typing in responses on your computer.

2) Choose File > "Save As" (or Save a Copy, depending on the PDF reader) and save it somewhere easy to find, such as the Desktop.

3) Give it a name such as Membership application.pdf.

4) Open your email and create a message to admin@cbms.org.uk.

5) Locate the saved PDF in the place you saved it (Desktop for instance), select it and add it as an attachment.

Send the email.
